Simple steps to make this
1)Download the psd file by using the following link Open this in Photoshop
2)Delete the layer which has the name (chandy).
3)Use text tool to add your name in the middle.
4)Use transform tools to adjust the text to fit in.
Edit - Transform - Distort (or) any other
TIPS:-
1)I used the pen tool to draw the designs on either side.
2)use an appropriate font for the name

FONT DOWNLOAD
Improvisation :-

- use your own imagination to come up with some cool tribal designs on either side using pen tool.
- or download some tribal design brushes and use it.
- add some glow effect using blending options to the text. (right click on layer - blending options - outer glow)
