Simple steps to make this
1)Download the psd file by using the following link Open this in Photoshop
2)Delete the layer which has the name (chandy).
3)Use text tool to add your name in the middle.
4)Use transform tools to adjust the text to fit in.
Edit - Transform - Distort (or) any other
